Getting There: Directions From Each City to University Drive

From Miramar
Miramar residents can take University Drive north through Pembroke Pines. If you're in east Miramar, near I-95 or US 441, Florida's Turnpike north to Griffin Road can save a few minutes during rush hour.
From Southwest Ranches
Southwest Ranches is quiet, and so is the drive. Griffin Road or Stirling Road east will bring you to University Drive without much fuss.
Before you leave the house: bring your Florida driver's license or state ID and double-check that the address on it is current. The state uses that address for your registry record, and a mismatch is the most common reason a new patient's application gets held up after the visit.
What Happens at Your First Visit
First visits aren't complicated, and they're not a test. Plan on spending a short while with us, a bit longer if you have a complex history.
- Check-in. We confirm your ID and Florida residency and go over a short intake form covering your symptoms, medications and past treatments.

What to Bring From Home
You don't need a folder full of paperwork to qualify. But a little preparation makes the visit smoother and gives the physician more to work with.
- Florida driver's license or state ID with your current address. This is required.
- Any medical records you have that relate to your condition. Hospital discharge papers, MRI or X-ray reports, notes from a specialist or a VA summary all count.
- A list of current medications, including doses. A photo of your pill bottles works fine.
